Ministry Questionnaires
The forms below are to be completed by all who serve in ministry. The Basic form is for all ministries except those serving in children or youth ministry. Those called to serve with children or youth are required to complete the Children/Youth form.
These forms are used to help ensure that those who serve in ministry are in agreement with Calvary Chapel's statement of faith and provide the foundation for our ministry screening policy designed provide a safe and secure environment for those children and youth who participate in our programs and use our facilities.
The forms are reviewed by the leadership of the church but are otherwise kept strictly confidential. Please download (save file to your computer), fill in form, sign it, and provide it to the leader of the ministry you are applying to or drop it off or mail it to the church office.
